Connected Health Innovation at a Large Healthcare Delivery System: A Case Study
At Partners Connected Health, research focuses on technology-enabled solutions that empower patients and providers to transform healthcare delivery and improve efficiency, quality and clinical outcomes. Working directly with patients and providers in a variety of settings, Partners Connected Health is gaining important insights about patient engagement, behavior change via mobile health, remote monitoring, social media and video-based interventions. Learn about two programs illustrating the challenges, opportunities and lessons learned: a hyper-personalized mobile app monitoring a patient’s health behavior patterns to guide custom coaching and improve engagement increased daily activity that correlated with improvements in blood pressure and glucose levels. Conversely, a medication adherence program using a mobile app and electronic pill bottle proved more challenging for patients with hepatitis C. Innovations include user-centered design, software development, clinical research, data science and analytics to create, implement and validate connected health solutions that cross the chasm.
